General Information

The Isleta Health Center now has an entrance and exit only, to enter the parking lot please use the northeast entrance and to exit please exit to the southeast.


The Health Center has purchased two exterior buildings, one is for the pharmacy drive up and the other for screening appointments, temp checks, and check-in.


Face masks are required at all times.


Please let the staff know immediately upon arrival if you are experiencing any of the following symptoms:

 

  • Fever or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea


Let the staff know if you have traveled out of state or traveled to a coronavirus "Hot Spot".

What is a hot spot?  A hot spot is not a public health term, but has been used to describe areas with a particularly large cluster of COVID-19 cases.

Hours of Operation

Monday through Friday - 8 a.m. until 4:30 p.m.

    

The Behavioral Health Clinic has resumed direct patient contact on Monday, June 8, 2020, with careful adherence to protocols as established by the Isleta Health Center to include:

  • Entry through Isleta Health Center main entrance only (temperature taken)
  • Patients must be on time for appointment and wear face masks
  • Appointments scheduled at 15-minute intervals in order for patients to sign in and maintain social distancing 
  • Only one patient allowed in lobby at any one time 
  • All Behavioral Health staff members maintain distance of at least 6 feet 
  • All Behavioral Health staff members wear masks
  • Offices and lobby sanitized before/after each patient encounter

COVID-19 Schedule

The following is in place until further notice:

  • Only Dialysis patients are transported. 
  • Each patient is transported individually due to 6 ft. social distancing.
  • All patients have their temperature checked by the driver before getting in the van.
  • Face masks are to be worn at all times.

  • Some visits can be done via telephone call  
  • In person visits require a mask at all times.  
  • Please arrive 15 minutes prior to your appointment time. 
  • You must check in at the east side of the clinic for a temperature check and screening, you will be directed where to park, and a member of the IHC staff will escort you into the building.
  • No visitors allowed unless you have a minor patient.
  • Only the patient with the appointment will be allowed into the building.  The only exception is and adult accompanying a minor patient

Same Day Appointments

  • Dental is no longer accepting walk-ins.  Please call at 8am to schedule a same day appointment.

Dental Services Provided

  • Emergency Care: Pain management, extractions
  • Routine Care: Exams, x-rays, cleanings, sealants
  • Restorative Care: Fillings, root canals
  • Periodontal Care: Scaling and root planning
  • Orthodontics: through AIDC -- Not taking new patients at this time

Health information department

Reminder from the Health Information Department at the Isleta Health Center

  

Medical Record requests need to be done in writing. A release of information form needs to be completely filled out.

  

  • Due to the COVID Pandemic, Release of Information forms can be emailed to the patient requesting Medical Records or picked up at the Drive thru tent at the front of the clinic.
  • Completed record requests will be mailed back to the patient or may be picked up at Isleta Health Center. 
  • As outlined by HIPPA, a signed consent is needed for each communication.

  

If you are needing Medical Records to be send to a Provider, please list the Provider information to include Name of Facility, Address and Fax information. Providers may also contact the Health Information Department and request information directly. 

MEDICAL CLINIC

Face mask must be worn at all times.

If a patient removes their mask while waiting for their provider their appointment may be cancelled.

PATIENT REGISTRATION

Updated Patient Registration Process

We are now doing drive through screening for appointments.  Please stay in your vehicle and wear your mask.  A nurse will come to your vehicle and take your temperature and ask you some screening questions pertaining to COVID-19. Once you have completed the screening the nurse will instruct you where to park for your appointment. 


Patient registration is now calling all patients to pre-register them for their appointments 1 to 2 days before their appointments. We will call and identify ourselves as Isleta Health Center patient registration. 


** If patient registration was unable to reach you prior to your appointment by phone then a patient registration representative will come to your vehicle and review the information below with you: 

We are asking patients to review and update their demographic information or their children’s information if the child has an appointment. The following information will be updated:

  1. Current mailing address and physical address  
  2. Current Telephone numbers – Very important we have your current phone numbers. This is how our providers are reaching their patients. 
  3. Emergency Contact information, Next of Kin information
  4. Employment information for self, spouse if applicable.
  5. Insurance information
  6. We will be asking that you update your Assignment of Benefits, Consent to Treatment, and Acknowledgement of receipt of notice if Privacy Practice. We will read this to you and ask for permission to sign on your behalf. 
  7. Current email address
  8. We will also request any documentation that the file is lacking such as Birth Certificate, SS card, Tribal ID card or CIB. Please bring the requested information to your appointment and we will make a copy for your file.

Announcements

 Masks are required to be worn at all times while on the Health Care Center grounds. 

Pickup and Drop-off

Pharmacy is currently offering pick up and drop off services outside located on the North-side of the Health Care Center.   Please remember you must have your mask on at all times even when you are in your vehicle to utilize pharmacy drive through services.

Refill Requests

Refill requests require a 24-hour turnaround time. Refills may take up to 72 hours for providers located outside the Isleta Health Care Center.  


The refill line 869-4590 is available 24 hours a day 7 days a week. Messages for pharmacy may also be left on the refill line for the pharmacy staff. 

Physical Therapy

The physical therapy clinic is currently providing one-on-one, in-person treatment sessions. However, the check-in process looks a little different. 


  • Patients are to remain in their vehicles in front of the PT clinic and call 505-869-4398 to alert the PT staff they have arrived.
  • The PT technician will meet the patient outside and complete a temperature screening. 
  • Patients are required to wear a face mask during the screening process and for the duration of their appointment; a patient is only permitted for treatment upon a satisfactory screening.
  • The physical therapy clinic hours have remained the same: Monday-Friday, 8-4:30.

purchase referred care (PRC)

  

Purchase Referred Care (PRC):

  • Due to COVID-19 and following Dept. of Health recommendations, staff members of PRC are not meeting with patients on a face-to-face basis.
  • Patients are required to call PRC for ER/Urgent Care notification within 72 hours. 
  • Patients are also required to call PRC for any scheduled appointments.     
  • Referrals are available for pick up at the clinic drive-thru area or can be mailed give the time limit of appointment date.
  • Patient bills may be dropped off at the clinic drive thru area


The PRC department is very busy during this time of COVID-19, and may be difficult to reach.   You are encouraged to leave a detailed message and phone number where you can be reached.    Please be assured that your call will be returned in a timely manner.
