 The Health Information Management Team can provide you copies of your health records. This includes health records, labs, immunizations, and diagnostic/tests and now replacement COVID Vaccine Cards.  It is understandable that you cannot always be present for Patient, Elder, or Minor care. Please see the following links for information on establishing Personal Representative or authorizing consent for children to be seen by the provider.
