The Health Center has purchased two exterior buildings, one is for the pharmacy drive up and the other for screening appointments, temp checks, and check-in.

Medical Record requests need to be done in writing. A release of information form needs to be completely filled out.
If you are needing Medical Records to be send to a Provider, please list the Provider information to include Name of Facility, Address and Fax information. Providers may also contact the Health Information Department and request information directly.
If a patient removes their mask while waiting for their provider their appointment may be cancelled.

Updated Patient Registration Process
We are now doing drive through screening for appointments. Please stay in your vehicle and wear your mask. A nurse will come to your vehicle and take your temperature and ask you some screening questions pertaining to COVID-19. Once you have completed the screening the nurse will instruct you where to park for your appointment.
Patient registration is now calling all patients to pre-register them for their appointments 1 to 2 days before their appointments. We will call and identify ourselves as Isleta Health Center patient registration.

We are asking patients to review and update their demographic information or their children’s information if the child has an appointment. The following information will be updated:
Masks are required to be worn at all times while on the Health Care Center grounds.
Pharmacy is currently offering pick up and drop off services outside located on the North-side of the Health Care Center. Please remember you must have your mask on at all times even when you are in your vehicle to utilize pharmacy drive through services.
Refill requests require a 24-hour turnaround time. Refills may take up to 72 hours for providers located outside the Isleta Health Care Center.
The refill line 869-4590 is available 24 hours a day 7 days a week. Messages for pharmacy may also be left on the refill line for the pharmacy staff.
The physical therapy clinic is currently providing one-on-one, in-person treatment sessions. However, the check-in process looks a little different.
