The Health Information Management Team can provide you copies of your health records. This includes health records, labs, immunizations, and diagnostic/tests and now replacement Covid Vaccine Cards.
It is understandable that you cannot always be present for Patient, Elder, or Minor care. Please see the following links for information on establishing Personal Representative or authorizing consent for children to be seen by the provider.

Only you and your personal representative has the right to access information in your health record. A health care provider may request information only as needed for treatment by what is called Provider to Provider access.

This document is required to be completed in its entirety and returned in person along with all the original documents listed below. Patient registration will review the application and the original documents, make copies and return original documents to applicant: (Check list is part of the document)
▪ Non-Isleta tribal members residing on Isleta Pueblo submit Permission to Reside on
Pueblo Letter issued by Isleta Governor’s office.
▪ Letter stating Community Ties to Isleta Pueblo.
Once the application is reviewed for completion and received by patient registration the application is logged and forwarded to administration for determination. Any application missing needed documentation will be returned to the applicant. Please allow administration some time to complete the determination process. If after a month you have not heard anything regarding the submitted application please call billing supervisor (505)869-4888 for status update.
